Our Raminiscence programme is a dementia health & wellbeing programmed, providing a range of distinct offers for people who are newly diagnosed, living with dementia and their carers. We are seeking to appoint a Dementia Officer to develop and deliver person-centred physical activity, reminiscence, wellbeing and social activities that help participants remain active, connected and independent.
Within this role you will provide support for people who are newly diagnosed, deliver ongoing person-centred support for people living with dementia and supporting carers through regular contact, peer connection and health & wellbeing activities. In addition, you will co-ordinate referrals and participant pathways, develop and maintain strong partnerships to strengthen referral pathways and promote the programme in local communities that may experience barriers to accessing dementia support.
You will possess demonstrable experience in supporting people living with dementia or adults with comparable health & support needs, along with experience of supporting physical and mental wellbeing through physical activity, social activity or community-based interventions. In addition you will have an empathetic nature to support individuals in identifying achievable health & wellbeing goals and a deep understanding of how physical activity, mental wellbeing and social connection can support people affected by dementia.
A key part of this role will be to develop new partnerships and strengthen existing relationships to increase referrals, extend the programme’s reach and provide co-ordinated support for participants and carers – therefore your exceptional communication skills and ability to build relationships at all levels will be essential for success in this position.
