Coaching
When registering for and participating in activities provided by Derby County Community Trust, it is understood that the Trust cannot be liable for any loss or injury that the applicant may sustain whilst attending the weekend, holiday, after school or lunchtime session.

Our health programmes will often require the disclosure of further personal information in addition to that which is listed in the section below.
Any medical data, including height and weight measurements, will be taken confidentially and in person wherever possible. Where personal data, including BMI, is asked for online, this is in line with the service we can subsequently provide as per our funding agreement. If you do not want to disclose this information online, our staff are happy to do this in person instead.
Photography and Video
A child is defined as a person under the age of 18. We want to provide event participants/parents/guardians with quality images of their achievements. This often means photographing minors. We cover events with the permission of the event organiser and that of the parent or guardian on all of our forms – both in print and online (please note that if you leave this section blank we will assume permission is not granted).
At times, Derby County Community Trust may wish to take photos or videos of teams or individuals over 18. We adhere to The FA and Premier League guidelines to ensure that any photos or videos taken are only of those who have the correct permissions, and that images are safe and respectful and used solely for the purpose for which they are intended, which is promotion and celebration of the activities of Derby County Community Trust.
Any participant or parent/guardian of a participant can request at any time for their permission to be revoked and this will be actioned with immediate effect
