In partnership with Greater Derby PCN we aim to help patients with Chronic Pain or long-term health conditions to lead more active lifestyles.

The programme aims to support patients within Derby to take part in tailored physical activity to support them to live with a long-term condition. Sessions are exclusively for patients of registered surgeries.

Programme aims:

• Reducing daily pain.
• Improving lifestyle and diet choices.
• Aiming to reduce prescribed medication through alternative strategies.
• Shared discussion within a safe and support environment.
• Holistic approach to living with pain.
• Exit routes with DCCT Health and Wellbeing programme and supporting partners.
