Cardiac rehabilitation phase IV aims to get people back to as full a life as possible after a cardiac event such as a heart attack, heart surgery or stent procedure.

An aspect of cardiac rehab focuses on reducing risk factors. This includes improving lifestyles, such as diet, stop smoking and increasing physical activity levels. Cardiac rehabilitation also gives people the opportunity to meet other people who are going through similar experiences.

Referrals should be made by medical practitioners only using the form below.

Forms must be accompanied by a BACPR form which should be sent to jon.fairbrotherCJMS@dcct.co.uk. Paperwork must be returned before patients can join the programme.

  • Recent Complicated Myocardial Infarction
  • Unstable Angina
  • Uncontrolled Ventricular Dysrhythmia
  • Uncontrolled Dysrhythmia Compromising Cardiac Function
  • Third Degree Heart Block
  • Aortic Stenosis
  • Aneurysm Including Ventricular Aneurysm
  • Acute Illness
  • Thrombophlebitis
  • Thrombo-embolism
  • Acute Congestive Heart Failure
  • Significant Emotional Distress
  • Pericarditis and Myocarditis
  • Recent ECG changes suggesting other Acute Cardiac events
  • Severe Hypertension (180/110+)

Manpreet underwent her second open heart surgery in November 2023 and after completing the beginner phase 3 cardiac rehab at Derby Royal Hospital, joined the Trust’s 6-month Cardiac Rehab programme in May 2024.

Even though my surgeon and the surgery saved my life, I feel like the rehab and working with Jon has given me my confidence back and I have noticed an improvement in my fitness for sure.
